What do I need to know when I go fill out my forms?

Your cooperation in providing information, verifications and complying with program requirements is greatly appreciated and will help us to process your application as quickly as possible. If you have any questions regarding any of the information requested, make sure to ask your worker.

Please complete your statement of facts and other forms that were given to you, and know that you will be required to provide proof of the information on the forms. Make sure that each "yes" or "no" question is checked in the right box. If the answer is "yes", please fill in the appropriate blanks. Do not sign the forms before coming in: they must be signed in front of your worker.

What other information will I need to provide?

You will need to provide the following:

• Picture ID for all adults
• Social Security cards for everybody
• Birth certificates
• Immigration entry card/Alien ID card
• Pregnancy verification with due date
• Doctor statements
• Life insurance policies
• Bank statements
• Auto registration slips on all vehicles owned or being purchased
• Housing statements or rent receipts
• Utility bills
• Check stubs
• Unemployment benefit verification
• Social Security verification
• Immunization records

Other forms may be given to you at the interview, or other verifications may be requested. You will be given a new date to provide the additional information if needed.
