Quality Improvement Work Plan - 2009-2010

Each year, Tulare County Mental Health develops a Quality Improvement (QI) Work Plan that serves as a guide in the identification and remediation of specific issues that the System of Care may encounter. As issues are identified from previous Work Plans or that are identified throughout the year, the Work Plan establishes activities and outcome measures in the resolution of the issues identified. QI Work Plan activities are also designed to monitor and evaluate the appropriateness and quality of services to individuals, pursue opportunities to improve services, and to initiate two performance improvement projects that will contribute to meaningful improvement in clinical care and beneficiary services.

The QI Work Plan shall include the following:

A. An annual evaluation of the overall effectiveness of the QI Program, demonstrating that QI activities, including performance improvement projects, have contributed to meaningful improvement in clinical care and beneficiary services. The annual review will also describe completed and in-process QI activities, including performance improvement projects.

B. Monitoring previously identified issues, including the tracking of these issues during the fiscal year.

C. Planning and initiation of activities for sustaining improvement in service quality and delivery.
