The Child Health and Disability Prevention (CHDP) Program provides health checkups by trained medical staff to:
- Find problems early
- Help people stay healthy
- Answer health questions
You are eligible if you are:
- On Medi-Cal (birth to 21 years)
- Income eligible (birth to 19years)
- Many working families qualify for these exams
Your child may receive these checkups from a private physician or a public health clinic.
The choice is yours.
All CHDP Check-ups Include:
- Hearing test
- Vision test
- Teeth & Gum check
- Immunizations (shots)
- Growth Development check
- Lab tests
- Health information
- Nutrition check
THE CHILD HEALTH AND DISABILITY PREVENTION PROGRAM (CHDP)
provides these checkups at no cost to you.
