The Linkages Program helps avoid inappropriate or premature placement of functionally impaired adults, 18 years of age or older, and/or frail elderly adults in nursing facilities. Also to foster independent living for those adults who may be safely discharged from nursing facilities, or for those who are at risk for placement in nursing facilities. The Linkages Program was designed to provide assistance for those clients who would not normally be eligible for other case management services due to age, income and disability criteria.
