What role does Public Health Nursing play?
The Public Health Nurse receives the initial referral on all infants meeting the eligibility criteria for the High-Risk Infant Team. A Public Health Nurse makes a home visit to assess the current physical and developmental status of the infant, and current needs of the family. This information is given to the High-Risk Infant Team if the family consents to referral.
What services does Public Health Nursing provide?
- Ongoing assessment of health and psycho-social needs.
- Counseling and education on the care of a child with special needs.
- Periodic growth and developmental screening to assess if the infant is progressing at expected levels.
- Referral to appropriate agencies for needed services.
- Coordination of services with other providers involved with the child and the family.
